Throughout the 6-month study duration, you’ll be required to document your journey through periodic video recordings. Rest assured, these videos will be shared exclusively on UVTREAT’s website and social media channels for educational purposes, ensuring privacy and confidentiality.

• Study Duration: The study will span 6 months from the date of device receipt.
• Medical History: You are required to provide details of your medical history, including your
  condition, diagnosis date, skin type, and past treatment experiences.
• Treatment Providers: Kindly share the names of clinics and medical providers from whom you have
  previously sought treatment.
• Progress Tracking: Regular video recordings every 2 weeks will help us monitor your progress.
  Additionally, staying in touch with our team for periodic check-ups is essential.
• Device Provision: Our device will be tailored to your individual needs and provided to you at no cost.
• Warranty Validation: For warranty purposes, we require your credit card information and billing
  address. A nominal $1 charge will be made for card validation, with no additional fees incurred.
• Agreement Dismissal: Should you wish to terminate our agreement, you must return the device
  promptly. Failure to do so will result in the full cost of the device being charged to your card.
• Study Conclusion: The study will conclude upon either achieving full recovery or reaching the end of
  the 6-month period, whichever occurs first.
• Device Ownership: Upon study completion, the device is yours to keep, offering continued support
  for as long as you need.
